1. Not a Substitute for Medical Care
The wellness services offered by Revitalyfe Wellness, including concierge mobile IV hydration therapy and vitamin/wellness injections delivered at your location, are elective services intended to support general wellness. They are NOT a substitute for professional medical advice, diagnosis, or treatment.
If you have a medical condition, are pregnant, nursing, or taking prescription medications, please consult your licensed healthcare provider before receiving any of our services.
2. Good Faith Examination Requirement
In compliance with California law and applicable medical regulations, all clients are required to complete a health intake screening prior to receiving services. Our supervising physician or licensed provider must conduct or review this screening and provide approval before any service is administered. This constitutes a good faith health review; it does not establish a comprehensive physician-patient relationship.
3. Provider Approval
Our services are administered under the oversight of a licensed and supervising medical director. All service protocols are reviewed and approved by our supervising physician. We reserve the right to decline service to any individual for whom we determine the service is medically inadvisable, based on intake information or clinical judgment.
4. No Medical Diagnosis or Treatment
Nothing provided by Revitalyfe Wellness — including consultations, health intake screenings, service selections, or any written or verbal communications — constitutes a medical diagnosis, medical treatment, or prescription. Our providers do not diagnose, treat, cure, or prevent any disease or medical condition.
5. Informed Consent
Before receiving any service, you will be asked to review and sign an Informed Consent form. By signing, you acknowledge that:
- You understand the nature of the service and its potential benefits and risks.
- You have disclosed all relevant health information accurately and completely.
- You consent to receive the requested service.
- You understand that results may vary and are not guaranteed.
6. Potential Risks
IV hydration therapy and injectable vitamin treatments carry potential risks, including but not limited to:
- Bruising, swelling, or discomfort at the injection or IV site
- Allergic or hypersensitive reactions
- Infection (though rare with proper sterile technique)
- Overhydration or electrolyte imbalance
- Nausea or lightheadedness
Our providers follow strict sterile and clinical protocols to minimize these risks.
7. Emergency Instructions
If you experience a medical emergency during or after your service, call 911 immediately or proceed to the nearest emergency room.
Signs of a serious reaction may include: difficulty breathing, chest pain, severe dizziness, facial swelling, hives, or loss of consciousness.
