Revitalyfe Wellness

Effective Date: June 9, 2025

1. Overview

Revitalyfe Wellness is a concierge mobile wellness service offering IV hydration therapy and vitamin injection packages, multi-session bundles, and promotional programs designed to provide clients with enhanced value and ongoing wellness support. By purchasing any package or participating in any promotional program, you agree to the terms set forth in this document and in our Terms of Service.

2. Buy 3 Get 1 Free — Drip Package

2.1 Description

When you purchase three (3) qualifying IV drip sessions at the regular single-session price, you receive one (1) complimentary IV drip session of equal or lesser value at no additional charge.

2.2 Eligibility & Redemption

  • Available to all clients who have completed a health intake screening and received provider approval.
  • All four (4) sessions (3 purchased + 1 complimentary) must be scheduled and completed within six (6) calendar months from the date of initial purchase.
  • The complimentary session is of equal or lesser value to the lowest-priced session in the package.
  • Sessions may be booked individually within the validity window.

2.3 Transferability

Package sessions are non-transferable. Sessions may only be used by the purchasing client and may not be gifted, sold, or transferred to another person.

2.4 Expiration

All unused sessions expire six (6) months from the date of purchase. Expired sessions are forfeited with no cash value and are non-refundable. No extensions will be granted except at the sole discretion of management in documented extenuating circumstances (e.g., medical emergency).

2.5 Refunds

Once purchased, the Buy 3 Get 1 Free package is non-refundable. If you have not used any sessions, you may request a credit toward a different package within 14 days of purchase. Partially used packages have no refund value for unused sessions.

3. Individual Drip Packages

The following packages are available. Pricing and session counts are disclosed at point of sale:

  • Revitalyfe Drip Package — $525 (Save $175)
  • Liquid Revival Package — $735 (Save $245)
  • Reinforcement Package — $705 (Save $235)
  • Luminous Lyfe Package — $795 (Save $265)
  • Vital Armor Package — $720 (Save $240)
  • Lyfeline Package — $885 (Save $295)

All packages are subject to the following general terms:

  • Valid for six (6) months from date of purchase unless otherwise stated.
  • Non-transferable. For use by the purchasing client only.
  • Non-refundable after use has begun.
  • May not be combined with other discounts unless expressly stated at checkout.
  • Subject to availability. We reserve the right to modify package contents with notice.

4. Referral Program — 10% Off

4.1 How It Works

When you refer a new client to Revitalyfe Wellness and that client completes their first paid service, you will receive a 10% discount credit applied to your next single-session service booking.

4.2 Terms

  • The referring client must be identified at the time of the referred client’s booking (name and contact details).
  • The referral credit is issued after the referred client’s appointment is completed and payment is confirmed.
  • The 10% credit applies to a single service and cannot be combined with other promotions or package pricing.
  • Referral credits expire 90 days from the date of issuance.
  • Credits have no cash value and are non-transferable.
  • There is no limit on the number of referrals you may make, but credits may not be stacked on a single service.

5. General Package Conditions

  • All packages require that you maintain an active health intake screening on file. If significant health changes occur, a new intake may be required before services are rendered.
  • We reserve the right to discontinue or modify any package or promotional program with 30 days’ notice to active package holders.
  • Package pricing reflects the value at time of purchase. Price increases after purchase do not affect existing packages.
  • Packages are void where prohibited by law.

6. How to Redeem

To redeem package sessions or apply credits, book your appointment through revitalyfewellness.janeapp.com or by contacting us directly. Reference your package at time of booking. Our team will verify your balance prior to your appointment.
